No sub-contract work can be assigned or commissions paid until all sub-contractor screening requirements are satisfactorily completed and all sub-contractor documents are signed by both parties and approved through FMG Corporate Headquarters. Simplified how to: Every FMG sub-contractor applicant must submit pages 1 thru 5, plus provide digital scans of their current motor vehicle driver license, social security card, and motor vehicle insurance card. Listing of Contents: Sub-Contractor Information Form, Pre-Employment Inquiry Authorization Release Form - for Vendor Background Screening, Commissions Retention Agreement, Independent Contractor Acknowledgement & Non-compete Agreement, Liability Waiver Agreement,  Customer Satisfaction Statement. Mandatory documents: (unable to process application without legible copies of these documents) Digital Scan of sub-contractor applicants Driver License or State issued photo ID Digital Scan of sub-contractor applicants Social Security card or Passport/Visa Digital Scan of sub-contractor applicants Proof of Motor Vehicle Insurance. Application Process Applicants should submit a copy of the completed applications via email to their project manager. Project managers then must send the original application in their next weekly package to the Kentucky office along with their other weekly payroll submissions. Upon satisfactory completion of the mandatory background and drug screens, written notice of satisfactory completion will be sent out via email to the RMH Project Manager certifying that the Sub-contractor Applicant is approved. All completed file documents will be sent to the Kentucky office for archival storage.

Independent Contractor Acknowledgement and Non-Compete Agreement

This Acknowledgment of Independent Contractor (the "Agreement") is made and effective as of this date listed.

BETWEEN: Frontline Marketing Group LLC. ("FMG" the Company), a corporation organized and existing under the laws of the State of Texas, with its head office located at: 7301 Ranch Road 620 North, Suites 155, 197, Austin, Texas 78726 AND the Sub-contractor/Applicant

NOW, THEREFORE, in consideration of the mutual agreements and covenants herein contained the Contractor hereby acknowledges that it has been retained by the Company, for purposes of: Performing assigned services (auditing, marketing, sales, installations collections) of subscriber accounts within specific operator system(s) as prescribed by the Terms & Conditions of the master contract between the Contract Operator and the company. In consideration of the foregoing, the Company agrees to pay the undersigned payment as follows: 1099 Weekly Commissions (subject to 20% retention for Operator Invoice charge backs)

  1. The undersigned shall be deemed an independent contractor and is not an employee, partner, agent, or engaged in a joint venture with the Company.
  2. Consistent with the foregoing, the Company shall not deduct withholding taxes, FICA or any other taxes required by an employer and I acknowledge my responsibility to pay such taxes as an independent contractor.
  3. I further acknowledge that I shall not be entitled to any fringe benefits, pension, retirement, profit sharing or any other benefits accruing to employees.

For a period of two years following termination of this Agreement, the Contractor shall not, directly or indirectly, through services to any partnership of which Contractor is a partner or employee or through any corporation or other entity in which Contractor has any interest or by whom is employed, compete with the Company or any of its affiliates or subsidiaries in any City, in any activity in which the Company or its affiliates or subsidiaries may have been engaged within two years prior to the termination of this Agreement.

  1. In the event of an actual or threatened breach by the Sub-Contractor of the provisions of this paragraph, the Company shall be entitled to injunctive relief restraining the Sub-Contractor from the breach or threatened breach. Nothing herein shall be construed as prohibiting the Company from pursuing any other remedies available to the Company for such breach or threatened breach, including the recovery of damages from the Sub-Contractor.​
